Tóm tắt
436 bệnh nhận với 506 khớp háng đã được phẫu thuật thay khớp tại Khoa CTCH viện 103
từ 1998 đến 09/2008. Nam: 321, Nữ: 185. Tuổi trung bình lúc phẫu thuật 61,2 (Từ 22 tuổi
đến 90 tuổi). Hầu hết các bệnh nhận được chỉ định thay khớp là do bị thoái hoá khớp và hoạt
tử tiêu chỏm xương đùi (59,5%); Số còn lại là gãy cổ xương đùi ở người già, viêm cột sống
dính khớp, u xương vùng cổ xương đùi…Thay khớp háng toàn phần được chỉ định cho 340
trường hợp, thay chỏm A.Moore hoặc chỏm Bipolar cho 166 trường hợp.Từ tháng 07/2005
áp dụng kỹ thuật thay khớp hang với kỹ thuật can thiệp tối thiểu cho những trường hợp có chỉ
định(116 BN với 168 khớp đã được thay theo kỹ thuật này). Kết quả theo dõi từ 6 tháng đến
7 năm (trung bình 3,6 năm) ở 360 bệnh nhân theo tiêu chuẩn của Merle d’ Aubigné – Postel
cho thấy: Kết quả rất tốt: 41,6%, Tốt: 45,2%, Khá: 11,6%, Kém: 1,6%.Các biến chứng đã gặp
bao gồm: 1trường hơp tử vong sau phẫu thuật, 3 trường hợp nhiễm trùng ,3 trường hợp gãy
xương đùi,3 trường hợp lỏng chuôi khớp,2 trường hợp sai khớp. Nhóm bệnh nhân được thay
khớp với kỹ thuật can thiệp tối thiểu không gặp biến chứng nào, kết quả bước đầu cho thấy
đây là kỹ thuật có nhiều ưu điểm.
Summary
436 patients with 506 hip replacements were performed at the hospital 103 from 1998 to
09/2008, Males: 321, Females: 185. Average age 61,2 (from 22 to 90 years old). Most of the
patients (59,5%) suffered from osteoarthritis and avascular necrosis of the femoral head, the
remaining includeds old fractures of the femoral neck, ankylosing spondylitis, tumors…Total
hip replacements were performed in 340 hips, A. Moore and Bipolar prothesis were 166 hips.
From 7/2005 started minimally invassive hip replacement (116 patients with 168 hips were
performed this technique) Results following from 6 months to 7 years (average 3,6 years)
according to Merle d’ Aubigné – Postel in 360 patiens were excellent: 41.6%, good: 45.2%,
fair: 11.6%, bad: 1.6%. There were 3 infections for an incidences 0,59%, one of these were
late infection, loosening of the femoral component were 2 patients, fracture of the femur were
3 patients, postoperative were 1patent.The patients with minimally invasive hipreplacement
were no complication,early results are exellent.
